ASIH membership data template (version 1.1 - revised 8 Sept. 1995) We ask that members submit requests to change or add information to their database records by using the form available for this purpose on the ASIH World Wide Web server if possible. This file is provided for those unable to access the WWW or with browsers that don't support forms. This file should be edited by adding your membership data changes following the field headers (e.g. "from:") below. Do not delete, edit or move any of the field headers. Field header names are self-explanatory or explained below. A sample, completed form is provided below, followed by a blank form. You need only return the completed blank form. After completing it, e-mail your form to smowry@allenpress.com as the body of a normal e-mail message. Copy the text from the subject line to the subject line of your e-mail message.
